Privacy Policy
How we collect, use, and protect your information across our website, studio, and client portal.
5th Ave Fitness LLC ("5th Ave Fitness," "we," "us") respects your privacy. This Privacy Policy explains what information we collect from clients, prospects, and website visitors, how we use and protect it, and the choices available to you. It applies to our website, our client portal, our in-studio intake process, and our payment systems.
1. Information We Collect
Contact and account information
Name, email address, phone number, and mailing address, collected when you book a consultation, purchase a package or membership, or create a portal account.
Health and fitness information
Medical history, current medications, physical limitations, blood pressure readings, and related information you provide on our Health History Questionnaire and Informed Consent form, along with progress notes, body measurements, and assessment results recorded by your trainer.
Payment information
Purchases are processed by Stripe, our third-party payment processor. We do not store your full card number on our own systems — Stripe handles and secures that data directly. We retain records of what was purchased, the amount charged, and billing status.
Photos and video
We only capture progress photos, video, or images of you for marketing purposes with your separate written consent, obtained before any such image is taken or used.
Website and portal usage information
When you visit our website, we use Google Analytics and Google Ads conversion tracking to understand site traffic and measure the effectiveness of our advertising. This may include your IP address, browser type, pages visited, and referral source, collected via cookies and similar technologies.

3. Confidentiality of Health Information
Health and fitness information you disclose to us is used solely to design and monitor your exercise program and will not be released to any third party without your written consent, except as required by law. 5th Ave Fitness is a personal training studio, not a healthcare provider, and is generally not a HIPAA-covered entity; however, we apply comparable safeguards to protect the sensitivity of this information.
4. How We Protect Your Information
Physical intake forms containing health information are stored in locked cabinets on-site. Electronic client records are stored using encrypted storage systems. Access to client health and payment information is limited to staff who need it to deliver your training program or manage your account.

7. Data Retention
We retain client records, including health history and session information, for as long as your account is active and for a reasonable period afterward to comply with legal, insurance, and business record-keeping requirements.
